WELCOME
It’s an honor to welcome you to the 2016 NIC Spring Investment
Forum. As our industry continues to progress at a particularly
rapid pace, it is now more crucial than ever to create business
and growth strategies that set up your organization for success
today and in the future. We are excited to connect with many of
you over the next three days, as we network and explore strategic
capital and business options.
If you’re seeking capital, be sure to attend Capital Connections
on Wednesday. Launched in 2015, Capital Connections has
been revised with a more informal, conversational format to help
facilitate meaningful discussions between capital seekers and
select capital providers on various financing options available to
address specific capital requirements.
In addition to networking, the Forum’s education sessions will
address a wide range of issues—from the market’s challenge to
absorb the growing number of new units coming online to the
impact of the rapidly evolving health care payment and delivery
model for seniors. This year’s highlights include:
• D an Mendelson’s keynote on how the changing health care
reimbursement and delivery model and access to data are
driving changes in all aspects of seniors housing and care
• R ichard Fisher’s discussion of what’s happening in the economy
and what changes could mean for the availability and cost of
capital
• V olunteer leaders of the industry’s major trade groups—
Argentum, AHCA, ASHA, and LeadingAge—speaking to what
they are doing within their own companies and their associations
to prepare for the changes being driven both by a new and
more demanding consumer and by the new health care delivery
model for seniors
We encourage you to take advantage of the Forum’s offerings to
help you provide outstanding seniors housing and care now and
in the future. Thanks for joining us. Have a great Forum!

• determine where we are in the construction cycle
• assess which markets appear to be overheated

Participating in a Care Coordination Model
(Level Three—Dallas Ballroom G)
The skilled nursing industry has experienced transformative
change in recent years as it transitions from a focus on long
term care to intensive, rehabilitative transitional care. At the
same time, payor sources and methodologies for payment
determination are changing rapidly as well. Owners and
operators of skilled nursing communities must implement
strategies to adapt to the changing payment landscape if they
are to survive. But how and where do they begin?
Know the steps you need to take to participate in—and
benefit from—a care coordination model. Industry experts will
share concrete examples of how risk sharing affects business
models and value, and explain bundled payments and their
impact on organizational returns.
Key takeaways from this session include:
• financial and organizational impacts and benefits of
participating in a care coordination model
• best practices for incorporating care coordination into your
business model
• tips for implementing and managing bundled payments
• risk mitigation approaches for bundled payments and risk
sharing

Show Me the Data: Technology in an Outcomes-Based
Industry
(Level Three—Dallas Ballroom E)
As the digital revolution continues to change how industries
around the world operate, the pressure on seniors housing and
care properties to adopt technology solutions will only increase.
It’s no longer about “show me the money.” It’s now “show me the
data”—data that proves you are producing quality outcomes and
achieving operational success.
A panel of industry leaders will share their experiences with
various technology initiatives and how they have integrated
technology across their organization—from the clinical side, to
HR, marketing, and finance. They will address what worked well,
where technology went wrong, and what you need to do to set
your technology strategy.
This session will cover:
• programs that work and the outcomes that matter
• how standardized metrics will measure performance going
forward
• benchmarks organizations will have to hit to succeed

Shock to the System: Surviving & Thriving in a Rapidly
Changing Seniors Health Care Model
(Level Three—Dallas Ballroom D)
With massive and fast-moving changes occurring in the health
care delivery and payment system, and an increasing focus on the
health care spend for seniors, unprecedented opportunities and
challenges are arising for seniors housing and care—from assisted
living and memory care, to post-acute and long term care. What
are the new expectations, requirements, and opportunities for
seniors housing and care operators and investors? And what must
they do to prepare not only to survive but thrive?
Keynote speaker Dan Mendelson will share his perspective on
how the changing health care reimbursement and delivery model,
coupled with the explosion of access to data, is driving and will
continue to drive changes in all aspects of seniors housing and
care. Enormous challenges exist, but there is also a tremendous
amount of opportunity for operators who can leverage data
insights to provide a consumer-driven experience that delivers
quality outcomes in the lowest cost setting.
Following the keynote, a discussion featuring the conference chair
and co-chairs will be moderated by NIC CEO Bob Kramer. The
panel of senior living operators and capital providers will respond
to Dan Mendelson’s comments and discuss how they see the
seniors housing, skilled nursing, and post-acute sectors and their
capital providers responding to these changes.

Improving Quality with Technology: Not Someday but Now
(Level Three—Dallas Ballroom F)
It’s often said that technology will shape the future of aging—that
someday, new technologies will change the delivery and quality
of care. But in reality, “someday” is already here. Organizations
today are integrating solutions that improve both quality of life for
the residents and operational efficiencies for their communities.
Looking to implement new technology is just as crucial for your
one-year plan as your ten-year, as more of your competitors will
have it and more of your residents/families, team members, and
business partners will demand it.
This session will help you discover how technology provides
opportunities in marketing, staffing, outcomes documentation,
and quality improvement that can advance the bottom line of your
business now and in the future.
Key takeaways include:
• why tech-enabled systems are becoming a “must have” and
are more than a cool gadget
• how and why senior care is evolving through the use of
technology
• how technologies are being used today for improving
efficiencies and outcomes
• the benefits of utilizing these systems for the operator,
investor and resident
• how these tech-enabled systems are being used during
pre-opening marketing and at existing communities to give a
competitive advantage for market share

Provider Perspectives on the Changing Seniors Health Care
Landscape
(Level Three—Dallas Ballroom E)
The ever-changing landscape of health care in America has
influenced and will continue to influence every player in seniors
housing, post-acute, and long term care—from the way health
care is delivered, to the way providers are paid. With nearly
every aspect of a facility’s daily operations under scrutiny, how
do operators navigate the changes and position themselves for
success today and going forward?
Understanding how the reimbursement, regulatory, and

Stepping Up to the Plate: Opportunities and Challenges
for Assisted Living in the New Integrated Health Care
Delivery Model
(Level Three—Dallas Ballroom F)
The new integrated health care payment and delivery model
for seniors presents assisted living with valuable opportunities
to evolve its market by serving as a coordinator and integrator
of care for residents who prefer the assisted living or CCRC
setting. In this session, Dr. David Grabowski will share results
from his research, which demonstrates the opportunities and
challenges assisted living and CCRC operators face in making a
contribution to the health care reform goal of improved outcomes
achieved in lower cost settings. Dr. Kevin O’Neil will present
results from Brookdale’s CMS Health Innovations Challenge
Grant, which focuses on improving care transitions from assisted
living communities, as well as reducing avoidable hospitalizations
and the overall cost of care. In addition, he’ll speak to the
steps providers need to take in order to participate in the new
integrated care networks for seniors.
Key takeaways from this session include:
• assisted living’s value proposition for seniors and their
families, healthcare providers, and payers
• the future of assisted living in a changing health care delivery
and payment model
• the cost benefits for payers—and potential revenue streams
for assisted living—that integrated care networks can bring
• what will be required of assisted living, CCRCs, and other
senior care providers in order to participate in the new health
care payment and delivery model
• what it will mean for AL, CCRCs, and other senior care
providers who choose not to participate in the emerging
seniors health care delivery model

What Skilled Nursing Investors Need to Know about
the Five-Star Quality Rating System
(Level Three—Dallas Ballroom E)
Reimbursement is rapidly moving from “volume” to “value.” As
a result, it’s becoming more essential than ever for investors to
evaluate the quality of care being provided by operators of skilled
nursing properties. Not only are payments being tied to quality,
but CMS and others are also using the Five-Star Quality Rating
System to serve as the gatekeeper for entrance by skilled nursing
properties into preferred networks, and the patient and revenue
opportunities they provide.
As an investor you need to understand:
• what the Five-Star Quality Rating System tells an investor
about an individual property operator
• how investors should both understand and use the Five-Star
Quality Rating System during due diligence
• what other quality assessment data should be included as
part of due diligence
• how CMS and others are using the system to determine
which skilled nursing properties are eligible to participate in
the new payment system
• how the revenue stream of the nursing home you own is put
at risk by its rating
• what happens when a property receives a major deficiency,
and how long it takes to recover
• the significance between a two-star and a three-star rating for
the viability of some skilled nursing properties
• the geographic variation in the rating system from state to
state, and the importance of careful benchmarking
• what rating is required for a skilled nursing provider to
be able to receive a waiver to the three-day hospital stay
requirement

Startups Powering the Shift from Reactive
to Proactive Care
(Level Three—Dallas Ballroom G)
Data is driving a shift from reactive to proactive care of seniors.
But what does this transformation mean to healthcare and senior
care? Where in the care continuum will the shift to proactive care
be most significant, and what barriers must be overcome to reap
its benefits?
This session will provide a practical and illuminating glimpse
into the data-driven care experience of the future. Aging2.0
partner and co-founder Stephen Johnston will moderate a
panel discussion with four startups who are leading the charge
in developing proactive care technologies for seniors and
the caregivers who support them. They will discuss how they
determined the need for their product and how their product
provides proactive care—from wearable technology and sensors,
to cloud-based platforms and telemedicine.

Market Momentum: Economic Trends and Their Significance
(Level Three—Dallas Ballroom D)
For more than seven years, the Federal Reserve kept interest
rates at historically low levels. Only recently did it remove its
foot from the brake and nudge interest rates higher. Cheap debt
fueled robust transactions activity as well as significant levels of
construction across all property types, including seniors housing.
Some analysts have been talking about possible asset bubbles.
Low interest rates also have impacted American seniors’ savings,
while the state of the overall economy also affects seniors’
decisions of when to move into a senior living community.
Keynote speaker Richard Fisher will provide his insights into why
the Federal Reserve has altered its policy and started to increase
interest rates and what factors likely went into their decision. He
will provide his view on where the economy stands today and
where it is headed in the future. He also will provide commentary
on what changes in the economy may mean to the availability and
cost of capital.
Richard Fisher is well respected for his ability to translate complex
economic terminology and concepts for investors at all levels of
experience. Delivered in an entertaining and relatable style, his
observations will provide attendees an understanding of the role
the economy plays in investment decision-making. Come join
us for this timely discussion on the economy, interest rates and
market reactions to the Federal Reserve’s decisions.

Coordinating and Collaborating Care: Why Seniors Housing
& Skilled Nursing Operators Need to Partner
(Level Three—Dallas Ballroom F)
Gone are the days when private pay seniors housing and skilled
nursing can operate in independent or even competitive silos.
Changes in the mindset of the consumer, reimbursements, and
expectations of health care providers for care collaboration are
forcing seniors housing and skilled nursing operators to rethink
the way they relate to each other. Successful business models
will require a tearing down of silo walls so you can partner with a
quality operator that complements the care you provide in order
to keep/gain market share.
Forming seniors health care delivery partnerships beyond the
walls of your community can enhance the marketability of both
partners and potentially improve census/income streams. In
this session, a panel of executives will discuss the opportunities
their companies gained by forming off-campus partnerships
and collaborating care. They will discuss why they formed the
partnership, what value was created, and what worked and didn’t
work in the formation.
The session will cover:
• why strategic partnerships are essential to gain market share
and enhance census
• why capital providers are attracted to this forward thinking
• how to begin the dialogue to create a successful strategic
partnership
• understanding the value and potential revenue streams
partnership can offer
• creating a referral source that minimizes lost referrals post-
discharge

The Growing Role of Ancillary Service Providers
(Level Three—Dallas Ballroom E)
Once an afterthought, ancillary service providers are now
an integral component of the delivery and continuity of care
within any community. With the changing competitive and
reimbursement environment, understanding the role these
services play is more critical than ever for meeting the needs of
your community, attracting and retaining residents, and enhancing
revenue streams.
Discover why ancillary service providers are becoming a
fundamental piece of the health care equation, as panelists
discuss how these services meet the clinical needs of seniors
housing and post-acute care operators’ residents. Gain an
understanding how payment reform (episodic and value-based
payments) will impact the operations and economics of ancillary
providers.
This session will detail:
• why the role of ancillary service providers is changing
• how integration of ancillary services can attract and retain
residents
• when to outsource these services and when to bring them
in-house
• how ancillary providers will support SNF’s management of

Synergistic Partnering: Creating Strategic, Mutually
Beneficial Partnerships
(Level Three—Dallas Ballroom E)
When does one plus one equal three? In partnerships formed by
diverse organizations coming together to produce highly creative
business approaches and opportunities that mutually benefit both
partners.
With the health care delivery model undergoing significant and
rapid changes, practices that were commonplace just a few years
ago are now outdated and must evolve to meet today’s needs.
Organizations that simply seek to repeat the successes of the
past may not have the same competitive edge tomorrow. In this
session, panelists who have formed partnerships and networks will
discuss evaluating alignment of mission, markets, and business
models, assessing risks and benefits, and adjusting expectations
based on actual results achieved. In addition, they will comment
on the results of their partnerships to date and what lessons they
have learned.

• creating new opportunities through collaborative effort
• identifying potential partners for your organization
• evaluating the competitive advantages and disadvantages
• determining business and reputational risks

Valuations in Today’s Seniors Housing & Care Market
(Level Three—Dallas Ballroom G)
Investor interest in seniors housing and care is strong. Transaction
volumes are high, and cap rates are near historical lows. That
said, the capital markets are in transition. How will a change in the

factors, such as rising labor costs, affect valuations through slower
NOI growth? And how do valuations for independent living
compare to those for assisted living and nursing care?
Hear directly from investors and capital providers on how they
view property valuation today.

How Capital Providers View the Emerging Care Continuum
(Level Three—Dallas Ballroom F)
The historically segmented approach to seniors housing and
health care is disappearing. Increasingly, operators across the care
continuum are working together to build integrated care models
designed to attract a new pool of consumers, diversify revenue
streams, and meet the complete health, wellness, and living needs
of residents. Capital providers are beginning to recognize the
value that a broader operational approach can play in driving the
real estate values within this emerging care model.
This session will include discussions about:
• investment parameters for evaluating care continuum
opportunities
• how the valuation of standalone investments and integrated
delivery models may differ
• valuation trends for integrated care models and the types of
investors who will become major players in the sector
• challenges of evaluating real estate development and
acquisition opportunities in the care continuum

The “New Normal” of MCOs in Long Term Care
(Level Three—Dallas Ballroom E)
Managed care organizations (MCOs) increasingly control more
of the patient population and are introducing new standards for
payment and reimbursement. As a result, assisted living and long
term care (LTC) providers are feeling the pressure to lower costs
and improve outcomes.
This “new normal” requires an understanding of the language, as
well as legal planning for the assisted living and LTC communities
to work successfully with MCOs. Experts in negotiating
relationships with MCOs from the operator perspective will share
relevant tactics to survive increasing financial and performance
standards in managed care contracting, billing, and legal risk
mitigation.
