These prices are effective as of January 1, 2021, and are subject to change without notice

The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral
arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not
specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

Professional Services

Our charge includes funeral counseling, necessary arrangements, recording vital statistics, securing permits, filing and obtaining death certificates and other forms and
claims, preparation of necessary notices, and coordination of service plans with parties involved in the final disposition of the deceased. This fee for our basic services
and overhead will be added to the total cost of the funeral arrangements you select. (this fee is already included in our charges for direct cremations, immediate burials
and forwarding or receiving remains.)

Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certai n
funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an

Direct Cremation

The charge for a direct cremation includes local removal, care of remains, transportation to crematory, basic services of funeral director and staff,
and authorizations (excludes ceremony, visitation, and crematory charges). If you want to arrange a direct cremation, you can use an alternative
container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside
covering). The minimum container we offer is cardboard. Prices exclude cash advance items.

This charge will be added to each item of a prearrangement that is guaranteed by the funeral home. Examples include, funeral home services, casket,

Payment Policy

To lose someone you love is a devastating human experience. Therefore, it is important to be realistic with respect to funeral
expenditures. We do not want your choices to be more than is affordable, nor less than you would like. Therefore, in an
effort to provide the finest services available, we are dependent upon the families we serve to make specific payment
arrangements prior to services and ceremonies. To avoid any possibility of misunderstanding, and in order to provide you
with financial flexibility, we offer the following payment options:

Application of funds from a prearranged burial reserve

When the financially responsible party resides outside of the Commonwealth of Pennsylvania, the funeral home
requires payment be made in full by the day prior to the scheduled services.

If the funeral expenses are to be paid from an estate, it is the responsibility of the person or persons signing the
statement of funeral goods and services to pay the expenses within thirty days.
