WHO WE ARE
For more than twenty years, Caring Health Center (CHC) has served as an essential
source of health care for residents of greater Springfield. When founded in 1995,
CHC employed 20 people and provided health care to 5,000 patients at one location.
Today, CHC employs 270 people, and serves more than 19,000 patients each year at
three locations. We have two designated locations that provide WIC Nutrition Programs
to Women and Children. In addition, the organization operates a pharmacy and serves as
the largest Refugee Health Assessment provider in Massachusetts.
CHC’s team of Community Health Workers, Health Navigators, and Interpreters ensure
that patients receive comprehensive care that addresses their cultural, economic, and
language needs. The Behavioral Health Specialists deliver services to address
a wide range of emotional and behavioral conditions.
CHC's Wellness Center provides nutrition education, group physical exercise, and other
services to encourage healthy and productive lifestyles among patients.

Healing Every L As President/CEO, my personal philosophy is coined in the acronym H.E.L.P.,
Healing Every Last Person. Your role in realizing/adopting this vision and being part of Caring
Health Center’s mission is deeply valued. Patient-centered; family focused care
is the core of our existence and delivering quality patient service is vital. Every person on
the team, whether an internal or external constituent, plays a critical role ensuring every
patient receives the best care possible.
Through collective effort and support, we strive to create an atmosphere enabling all to achieve their highest
level of excellence. With team based accountable care coordination we will move from Good to Great-
transitioning and transforming.

Patient Story
“This is a story about a current patient who first arrived at Caring Health
Center fourteen years ago, having traveled alone to the United States to start
a new life. With help from Jewish Family Services, the patient was set up for
her first refugee health visit, where she established a relationship with our
Refugee Health Coordinator. During her initial visit, the provider noticed some
abnormalities leading to a diagnosis of stage two breast cancer. The patient
bravely underwent chemotherapy and radiation treatment, while balancing
extreme hardships as she relied on public transportation for her treatments
and layoff from her job as a cleaner at a cafeteria. Fortunately, the patient
beat her diagnosis and became cancer-free. Unfortunately, last summer
when the patient received a routine mammogram, we discovered that
she again had stage two breast cancer, this time requiring surgery and
radiation therapy. Over the years, our providers have observed mood
changes and symptoms of depression. In response, the patient was
referred for services in our Behavioral Health Department – her first
experience receiving mental health care. This connection helped the
patient to find support in dealing with her high stress level related to
her medical, housing, and financial situation.
Besides medial and behavioral health support, the patient has also received
valuable social support at Caring Health Center. For example, one day the
patient arrived and shared that she had no money to pay for rent or food. Our
Community Support Program Workers and Refugee Health Coordinator helped
her obtain food stamps and cash assistance, and worked hard to help her
obtain an affordable apartment. Since then, these staff have paid visits to the
patient’s home and the patient has expressed that she considers our staff as
family. Recently, the patient discovered that her disability case was denied
and her health insurance was terminated, so our staff again advocated for
her by supporting her throughout the disability court process. Ultimately,
the patient obtained her disability and reimbursement. The patient
is currently cancer-free and continues to receive support from our staff.”

Patient Story
“My name is Ruthie and I want to tell you some of my recovery story. When I walked into Caring Health I was a broken woman
with no hopes or dreams, thinking that I was going to be an addict for the rest of my life. The drugs had hijacked my body for such
a long time that 13 yrs. passed and I felt there was no way out. That all changed with the help of the Team at CHC that worked
with me. When I walked in I wasn’t treated like an outcast and that meant a lot to me. I had tried to get help at other facilities but
never felt the heartwarming connection that I felt at CHC BH Dept. I was broken and they stood by me, like they really cared.
Their dedication helped me with my mental state and to just find myself mentally, emotionally, physically. I never had that
before, they helped me as a Team, they gave me choices and resources and options. They helped me work on myself. Therapy
helps me talk about all the pain I held inside for so many yrs. that led to my addiction. Their main concern was how I felt inside
and how I felt on the outside when I wasn’t in the program. In the groups I learned how to stay away from drugs, how to talk
about my feelings and turn a bad situation into a good one and that I don’t have to turn to drugs when something goes wrong.
They helped me find the Ruthie that I lost along the way with so many drugs in my body that hid the real me. I’m happy to share
that now I live happily because of the help they provided for me. I am closer to my kids and grandchildren. I have goals now, I
went back to school and I’m even thinking of my career now. If it wasn’t for their determination to help me I would be dead by
now, I weighed only 90 pounds. I was like a dead soul walking. The drugs sucked my life out of me. I feel that I’ve made a huge
change in my life but I wouldn’t have been able to do this without the guidance and support of CHC BH TEAM. I want to thank
everyone of the Team that helped in so many ways. There are no words to describe how much they have helped me to turn my
life around. Right now I’m in school and I also have a team in Drug Court that works with CHC for my goals and needs. They work
as a team for my future. I’ve never had anything like this before.
I’m so happy for the day I walked through these doors. It’s the best thing that has ever happened to me. It’s my 2ⁿd home. They
do this work because they care, not because it’s just a job. I have felt their concern and caring for me, as a person. They see me as
a human being who needs help and they give it. I want other people who are struggling with substance use to come to CHC BH
Department and get the help they need to recover, heal, and find themselves again, to create the life they always wanted but
didn’t have and never dreamed they could have. If I can do it, they can do it too! No one judges me in this program. They work as
a team with me and for me. Everything I needed is right here at CHC BH in one place and that made all the difference. I could get
what I needed all in one place. If I had to go to several different places and try to piece it all together I wouldn’t have been able to
do it, I would have gotten discouraged and given up. Today I feel hopeful, amazed at myself and my changed life, I feel strong and
happy and want others to know this too.“ ~Ruthie
Behavioral Health Services
At Caring Health Center, our Integrated Behavioral
Health/Recovery Services team believes that each
individual has unique medical, behavioral health
and recovery needs and deserves the most effective
treatment we can provide. We believe that providing
comprehensive, integrated care in a respectful, non-
judgmental environment allows each person served
to choose their healing journey and progress at their
own pace. We accomplish this through trauma-
informed, strength-based interventions for all
individuals seeking healing and support in
recreating their lives.

Wellness Center
Patient Story
“Hi. I/we would like to tell you about the Wellness Center at
the Caring Health Center in the South End of Springfield, MA.
There are 3 amazing instructors. Start off your week with
Melissa. You will have an awesome workout. Next is Yenitza
who will make you in awe and teach you about her fabulous
Zumba moves. Let me say Lamont will have you working out
in moves you never believed you could.
The best thing is each and every one of them care about you!!
They are concerned about your welfare and help modify the
moves if you have trouble with them. If a class workout is
not what you want there are treadmills, elliptical's, rowers,
weights and other things to utilize.
Believe me, you will choose the class �. Two other
amazing people are Jessica and Molly. If you need
help they can get you in the right direction or send
you to someone that will be willing to help.
There are beneficial programs there also,
Food Matters, how to deal with a chronic
illness, pre-diabetic and much more.

Wellness Center Service
CHC’s Wellness Center was established in 2009 in
response to NIH-funded research findings of high rates
of food insecurity and social/environmental barriers to
chronic disease management among CHC patients.
