5 • Three most important in abdominal
radiography are the diaphragm and the right
and left psoas major.
• The diaphragm is an umbrella-shaped muscle
that separates the abdominal cavity from
the thoracic cavity.
• AP supine abdomen / KUB
(kidneys, ureters and bladder).

Positioning for abdominal radiographs in AP
projections:
1. Xiphoid process (level of T9-T10)
2. Inferior costal (rib) margin (level of L2-L3)
3. Iliac crest (level of L4-L5 vertebral interspace)
4. Anterior superior iliac spine (ASIS)
5. Greater trochanter
6. Symphysis pubis
7. Ischial tuberosity

ความผดิ ปกติจากการรวมตัวของกาซ ในระบบทางเดนิ อาหาร
ก้อนนวิ่ ในทางเดินปัสสาวะ ถงุ นา้ ดี ตบั อ่อน กอ่ นการตรวจพเิ ศษตา่ ง ๆ เก่ียวกับช่องทองจะ
ก๊าชทร่ี วั่ เข้าไปในช่องทอ้ ง (peritoneal cavity) ต้องถา่ ยภาพเอกซเรย์ธรรมดา (Plain film
นา้ ในชอ่ งท้อง (Ascitis) รวมถงึ เลอื ดออกในชอ่ งทอ้ ง
ก้อนในชอ่ งทอ้ ง Abdomen) ในท่านอน และ/หรือ ทา่ ยืนก่อนเสมอ

11 ROUTINE : AP SUPINE PROJECTION
Patient Position :
Supine with midsagittal plane centered to midline
of table or IR.
Arms placed at patient’s sides , Legs extended.
Part Position :
Center of IR to level of iliac crests with bottom
margin at symphysis pubis.
No rotation of pelvis
(Both ASIS are the same distance from the tabletop)
12 AP SUPINE PROJECTION
CR :
CR perpendicular to and directed to center of IR (to level of iliac crest)
Technical Factors :
SID is 40 inches (102 cm).
IR size—14 × 17 inches, lengthwise
Grid
Digital systems—70 to 80 kV range
13 EVALUATION CRITERIA
Anatomy Demonstrated:
Outline of liver, spleen, kidneys, and
air-filled stomach and bowel segments
and the arch of the symphysis pubis
for the urinary bladder region.
14 EVALUATION CRITERIA
Position:
No rotation; iliac wings, obturator foramina (if
visible) and ischial spines appear symmetric.
Exposure:
No motion; ribs and all gas bubble margins
appear sharp.
Sufficient exposure (mAs) and long-scale
contrast (kV) visualize psoas muscle outlines,
lumbar transverse processes, and ribs.
15 SPECIAL : LATERAL POSITION: ABDOMEN
Patient Position :
Patient in lateral position on right or left side,
pillow for head.
Elbows flexed, arms up, knees and hips partially
flexed.
Part Position :
Align midcoronal plane with CR and midline of table.
Ensure that pelvis and thorax are not rotated but are in a
true lateral position.
16 LATERAL POSITION
CR : Center to perpendicular to table level of the iliac crest to midcoronal plane
IR centered to CR
Technical Factors :
SID is 40 inches
IR size—10 × 12 inches, lengthwise

Digital systems—80 to 90 kV range
17 EVALUATION CRITERIA
Anatomy Demonstrated:
•Diaphragm and as much of lower abdomen
as possible should be included.
•Air-filled loops of bowel in abdomen with
soft tissue detail should be visible in
prevertebral and anterior abdomen regions.
18 EVALUATION CRITERIA
Position:
•No rotation as evident by superimposition of
posterior ribs and posterior borders of iliac wings
and bilateral ASIS.
Exposure:
•No motion; rib and gas bubble margins appear
sharp.
•Lumbar vertebrae may appear about 50%
underexposed with soft tissue detail visible in
anterior abdomen and in prevertebral region of
lower lumbar vertebra.
19 SPECIAL: DORSAL DECUBITUS POSITION (CROSS-TABLE LATERAL)
Patient Position :
Supine position, side against table
Pillow under head, arms up beside head;
support under partially flexed knees.
Dorsal decubitus—right lateral position
20 SPECIAL: DORSAL DECUBITUS POSITION (RIGHT OR LEFT LATERAL)
Part Position :
Adjust patient center to midline of IR and CR is at
level of iliac crest (or 2 inches above iliac crest to
include diaphragm).
No rotation of pelvis or shoulders exists (both ASIS
should be the same distance from tabletop).
Adjust height of IR to align midcoronal plane with
centerline of IR.
Dorsal decubitus—right lateral position
21 DORSAL DECUBITUS POSITION (RIGHT OR LEFT LATERAL)
CR :
CR horizontal to center of IR 2 inches above iliac crest and to midcoronal plane
Technical Factors :
SID is 40 inches.
IR size—14 × 17 inches, crosswise

Digital systems—70 to 80 kV range
22 EVALUATION CRITERIA
Anatomy Demonstrated:
• Diaphragm and as much of lower abdomen as
possible should be included.
• Air-filled loops of bowel in abdomen with soft
tissue detail should be visible in anterior
abdomen and in prevertebral regions.
Dorsal decubitus—right lateral position
23 EVALUATION CRITERIA
Position:
• No rotation as evident by superimposition of

24 SPECIAL: LATERAL DECUBITUS POSITION (AP PROJECTION): ABDOMEN
Patient Position :
Patient lateral position.
Knees partially flexed, one on top of the
other to stabilize patient
Arms up near head; pillow provided
Left lateral decubitus (if patient cannot stand for
AP erect abdomen).
25 LATERAL DECUBITUS POSITION
Part Position :
Adjust patient center to midline of IR and CR are
approximately 2 inches above level of iliac crests
(Include diaphragm).
Proximal margin of IR is approximately at level of axilla.
Adjust height of IR to center midsagittal plane of patient
to center of IR
26 LATERAL DECUBITUS POSITION
CR :
CR horizontal, directed to center of IR at about 2 inches above level of iliac crest.
Note: Use of a horizontal beam to demonstrate air-fluid levels and free intraperitoneal air
Technical Factors :
SID is 40 inches
IR size—14 × 17 inches, lengthwise

27 EVALUATION CRITERIA
Anatomy Demonstrated:
• Air-filled stomach and loops of bowel and air-
fluid levels
• Should include bilateral diaphragm.
Position:
• No rotation; iliac wings appear symmetric
•Spine should be straight (unless scoliosis is
present) aligned to center of IR.
Left lateral decubitus (AP).
28 EVALUATION CRITERIA
Exposure:
• No motion; ribs and all gas bubble margins sharp.
• Exposure sufficient to visualize spine and ribs and
soft tissue.
29 SPECIAL: AP PROJECTION—UPRIGHT POSITION: ABDOMEN
Patient Position :
Upright, legs slightly spread, back against table or
grid device.
Arms at sides away from body.
Midsagittal plane of body centered to midline of
table.
Part Position :
Do not rotate pelvis or shoulders.
Adjust height of IR, the center is approximately 2 inches
above iliac crest (to include diaphragm),
Top of the IR approximately at the level of the axilla.
30 AP PROJECTION—UPRIGHT POSITION
CR : CR horizontal, to center of IR
Technical Factors :
SID is 40 inches (102 cm).
IR size—14 × 17 inches, lengthwise

EVALUATION CRITERIA Anatomy Demonstrated:
31 • Air-filled stomach and loops of bowel and air-
fluid levels.
• Should include bilateral diaphragm and as much
of lower abdomen as possible.
• Small free, intraperitoneal crescent-shaped air
bubble if present seen under right
hemidiaphragm, away from gas in stomach.
32 EVALUATION CRITERIA
Position:
• No rotation; iliac wings appear symmetric.
• Spine should be straight ,aligned with center of IR.
EXPOSURE:
• No motion; ribs and all gas bubble margins appear sharp.
• Exposure is sufficient to visualize spine and ribs and soft
tissue but not to overexpose possible intraperitoneal air in
upper abdomen.
