Over West’s takeout double, North applied the main conclusion from the Law of Total Tricks,
jumping to four spades because he knew of a 10-card fit. Dealer: South; Vulnerable: Both

play the bottom of his touching cards: the jack.

When South takes the trick with the ace, West knows his partner has the club queen. So,
when South next plays a trump, West can win and continue with a second low club to give
his partner the lead. Then, with luck, East will shift to the heart nine (the high card denying an
honor in the suit), and West will take two tricks in that suit to defeat the contract.

East should not lead to a diamond, because if West has a winner there, it isn’t going to
disappear. But a heart trick might — and will — evaporate.

Finally, if declarer ducks the first trick, East should immediately switch to the heart nine.
West’s lead marks South with the club ace, so returning that suit would be a waste of time.

Knee-to-know: Joint replacement not always best option

Orthopedic surgeon Dr. Ron Robinson
of Vero Beach’s Pro Sports Medicine &
Joint Replacement Center and Cleveland
Clinic Indian River Hospital has some
information you might need to know.

It’s entirely possible, Robinson says,
that you could be better off not replac-
ing your aching knee joint with a shiny,
new prosthetic version.

Wait a minute.
Doesn’t the American Academy of
Orthopaedic Surgeons call total knee
replacements “one of the most success-
ful procedures in all of medicine?”
Yes, it does.
And doesn’t the Agency for Health-
care Research and Quality say “more
than 600,000 knee replacements are per-
formed each year in the United States?”
That’s true, too.
Nevertheless, according to Cleveland
Clinic’s Health Library, “if you have re-
curring or chronic knee joint pain, you
may think joint replacement surgery
is your only option for relief. However,
you may want to explore several less

The mechanics of the knee joint are Still, with those 600,000 knee replace-
surprisingly straightforward.

The knee consists of the lower end
of the thighbone (the femur), the up-
per end of the shinbone (the tibia), and
the kneecap (the patella). The ends of
these three bones, where they touch,
are covered with articular cartilage, a
dense, smooth, durable substance that
protects the bones and enables them
to move easily, without friction, across
the adjacent bones.

But that protective cartilage can be
worn away or damaged by degenera-
tive arthritis (osteoarthritis) or injury.

If cartilage is still present, and knee
pain is not caused by bone rubbing
unpleasantly on bone, many knee
problems do not require joint replace-
ment to alleviate pain.

According to Cleveland Clinic, “even
in the case of osteoarthritis, surgery is
not the first choice. Whatever the cause,
you’ll want to preserve your joints for as
long as you can.”

The success of total knee joint re-
placements has led to a range of re-
placement procedures and products,
and many patients come in deter-
mined to have their joint replaced with
something new and high-tech.

“People come here and order them
like they’re at Baskin-Robbins,” Rob-
inson says. “‘I want this joint. I want
it done this way, and I want this, and I
want that.’ Advertising has made this a
whole new thing in orthopedics.”

And it’s true, Robinson continues, “in
a certain population, total joint replace-
ments are great, and they’ve come a
long way [but] … my thought is trying to
avoid replacements.

“We’ve got a population of patients
here that are just not candidates. They’re
too young. Even the best, greatest, latest

Statins, which were introduced in the
1980s, are the premier medicines for
high cholesterol and typically are the
first drugs prescribed along with chang-
es in diet and exercise. But they can
cause muscle aches and cramps, mak-
ing it hard for some people to take the
required dose, said Robert Rosenson, di-
rector of cardiometabolic disorders for
Mount Sinai Hospital in New York. More
than 10 percent of people taking high-
dose statins had muscle pain, according
to a 2005 study.

Because bempedoic acid targets an
enzyme in the liver, not in muscles, it
doesn’t cause muscle pain, he said.

Rosenson, who was not involved in
the development of bempedoic acid,
said the drug reduces cholesterol by
a relatively modest amount but could
nevertheless be helpful in achieving pa-
tients’ goals.

Recommended cholesterol levels vary
by age and gender, but the “optimal”
level for LDL cholesterol is 100 milli-
grams per deciliter or below, according
to the National Heart, Lung and Blood
Institute. Experts say high-risk people,
including those who have had a heart at-
tack, should aim for below 70 mg/dl.

The FDA approved bempedoic acid
based on its cholesterol-lowering ef-
fect. Whether the drug reduces the risk
of heart attacks and strokes is being
studied as part of a large outcomes trial
involving people who cannot tolerate
statins. The trial is scheduled to be com-
pleted in two years.

Bempedoic acid also can be used
with another class of heart drugs, called
PCSK9 inhibitors, which are injectable
medications launched in 2015. They can
reduce LDL cholesterol sharply when
combined with a statin. But they ini-
tially cost about $14,000 a year, causing
insurers to balk.

Since then, prices have fallen sharp-
ly, but uptake remains slow. While the
drugs “are highly effective, there are a
lot of hassles with prior authorization
and they often get denied,” said Christie
Ballantyne, a cardiologist at Baylor Col-
lege of Medicine in Houston who ran the
late-stage clinical trial for bempedoic
acid. 

Rechter said the plaza is about
a month out from getting final ap-
provals and he expects construction
to start by late summer. It will have
a courtyard in the center flanked by
two restaurants with indoor and out-
door seating.

Remaining space will be available
for other commercial or retail uses,
with five or six businesses total, ac-
cording to Rechter, who hopes to see
the plaza occupied and open by the
summer of 2021.

“U.S. 1 is coming along,” he said of
all the current and planned develop-
ment along the corridor. “I like new
construction. It’s very positive for the
area.”

Meanwhile, on the east side of the
highway, across from the new plaza
location, the shell of the Verizon store
is nearing completion and the busi-
ness is slated to open later this year.

The developer of “Lin’s Arcade” at
8th Street has leased the former pet
store property and is in the planning
and permitting process.

Jason Jeffries, Vero Beach planning
and development director, noted
that the proposed veterinary facility
doesn’t have an approved site plan
yet, but is working through the rezon-
ing process.

The developer is combining several
parcels that front U.S. 1 with land set
back to the west. Rezoning is neces-
sary for the parcels off U.S. 1 to allow
for stormwater features needed to
meet site development standards, ac-
cording to Jeffries.

“It takes more land than it used to”
under the current standards for a de-
velopment of this type, he said.

Billy Moss with Lambert Commer-
cial Real Estate says additional new
development will be coming to the
busy 1.5-mile stretch of U.S. 1. and
expects to announce additional proj-
ects in the future.

“A lot of people are looking,” said
Moss, who sees the 2017 opening of
Wawa as the turning point for the
area.
